多位专家呼吁：成立国家级罕见病基金

left\"\u003e\u003c!--AIPOS_0--\u003e中国高度重视罕见病防治与保障工作，近年来有关方在政策法规体系、临床诊疗水平、药品供应机制、社会保障体系等方面持续探索完善，不过还有一些呼吁和政策构想有待落地，其中最具代表性的当为成立\u003c!--VERTICAL_CARD_BEGIN_1--\u003e罕见病专项基金\u003c!--VERTICAL_CARD_END_1--\u003e。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e近一年来，包括对外经济贸易大学教授孙洁、浙江省衢州市中医医院副院长陈玮、湖北省咸宁市中心医院感染管理办公室主任王雪静在内的多位全国两会代表委员都曾提出成立罕见病专项基金的建议。在新一年国际罕见病日和全国两会来临之际，罕见病专项基金也是罕见病领域最受关注的话题之一。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e多位受访专家认为，只有建立\u003c!--VERTICAL_CARD_BEGIN_2--\u003e多方筹资\u003c!--VERTICAL_CARD_END_2--\u003e、多层次支付的保障机制，才能解决\u003c!--SECURE_LINK_BEGIN_0--\u003e罕见病\u003c!--SECURE_LINK_END_0--\u003e高值药的支付问题。其中，探索建立专项基金，是多层次医疗保障支付体系中的重要一环，也是解决罕见病保障“最后一公里”的重要路径。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e\u003cstrong\u003e地方探索\u003c/strong\u003e\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e\u003c!--AIPOS_1--\u003e与常见病不同，罕见病往往具有残酷的不可逆性，药物治疗往往伴随患者终身，且具有不可替代性。一旦因经济压力断药，患者将面临不可逆的身体损伤，致使前期巨大的治疗投入付诸东流。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e以\u003c!--VERTICAL_CARD_BEGIN_3--\u003e戈谢病\u003c!--VERTICAL_CARD_END_3--\u003e为例，目前的医保内药物有明确适应证限制。用于戈谢病\u003c!--VERTICAL_CARD_BEGIN_4--\u003e酶替代治疗\u003c!--VERTICAL_CARD_END_4--\u003e的药物价格非常昂贵，随着患儿每年体重增加，费用�万元�万元不等。对于仍需依赖酶替代药物治疗的患者，如果因支付问题导致断药，患者将面临不可逆的骨骼畸形和全身多脏器损伤。因此，建立稳定、可持续的资金来源是临床治疗持续有效的前提。\u003c!--MID_AD_0--\u003e\u003c!--EOP_0--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_0--\u003e\u003cp style=\"text-align: left\"\u003e上海是在罕见病防治事业上走在最前列的中国城市之一。2013年起，上海在国内率先建立戈谢病专项救助资金，与少儿住院基金、互助基金共同构建多层次保障机制，使患者自付水平显著下降，这是以专项基金作为突破口推动罕见病保障的一次典型实践。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e2025𻂋񀙑日，《上海市医疗保障条例》正式实施，明确将“鼓励社会力量参与罕见病的诊治、研究和用药保障”写入条款，为罕见病多元化保障体系的建立提供了法理依据。2026𻂊月，在该条例实施一周年之际，上海市政协原常委、上海市政协提案委员会原常务副主任黄鸣向经济观察报透露，上海浦东新区人大正在市人大的指导下，开展针对罕见病的立法调研。\u003c!--MID_AD_1--\u003e\u003c!--EOP_1--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_1--\u003e\u003cp style=\"text-align: left\"\u003e面对立法所需的探索周期，黄鸣提出“基金先行、立法补充”的路径：“专项基金本身应该是对立法工作的有益补充，我们应该首先把专项基金建立起来，通过运作不断总结经验、形成共识，进而丰富立法的内涵。这是一个从无到有的过程，通过先行先试，让专项基金的探索为未来的立法工作提供宝贵的实践样本。”\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e黄鸣说，上海有很多罕见病救助机制，但缺一个专项基金，真正要让多方支付能长久地、稳定地发挥作用，还是要进一步呼吁尽快建立专项基金。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e虽然上海没有罕见病专项基金，但其�年起通过戈谢病保障探索的\u003c!--VERTICAL_CARD_BEGIN_5--\u003e多方共担模式\u003c!--VERTICAL_CARD_END_5--\u003e，为破解罕见病高值用药保障难题提供了最早的“\u003c!--VERTICAL_CARD_BEGIN_6--\u003e上海方案\u003c!--VERTICAL_CARD_END_6--\u003e”。这一探索也为后续江苏省、浙江省、青岛市等部分经济较发达省市的罕见病政策探索提供了重要范本。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e此后，浙江省�年公布了《关于建立浙江省罕见病用药保障机制的通知》，建立了浙江省罕见病用药保障专项基金，迈开了罕见病专项基金的第一步，为其他地方建立罕见病专项基金奠定了基础，起到了示范效应。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e浙江的做法是，在省级医疗保险基金财政专户下设子账户，进行分账管理、独立核算。根据上年底基本医疗保险参保人数，按每年每𱍆元的标准，一次性从本统筹区大病保险基金中上解至\u003c!--VERTICAL_CARD_BEGIN_7--\u003e浙江省罕见病用药保障基金\u003c!--VERTICAL_CARD_END_7--\u003e。在报销时，以年为单位，0—30万元报销比例�%，30万—70万元报销比例�%，70万元以上的费用全额报销。同时，浙江将患者自付上限确定�万元。\u003c!--MID_AD_2--\u003e\u003c!--EOP_2--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_2--\u003e\u003cp left\"\u003e江苏省的做法则是，在医疗保障条例中精准、明确地提出建立罕见病用药保障机制，用药保障基金实行省级统筹、单独筹资，建立由政府主导、市场主体、社会慈善组织等参与的多渠道筹资机制。罕见病用药保障资金纳入省财政社保专户管理，专款专用，独立核算。在用药保障范围，对药品进行有效性评估、专家论证、价格谈判。\u003c!--MID_AD_3--\u003e\u003c!--EOP_3--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_3--\u003e\u003cp left\"\u003e罕见病药物研发成本高昂，患者群体基数小，这使得企业在研发过程中常考虑经济收益问题，王雪静认为，专项基金的设立能通过多元化的筹资渠道和政策支持，降低罕见病药物的使用成本，从而减轻患者经济负担。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e孙洁列举了罕见病专项基金的三个主要意义：一是有利于解决医保保障罕见病、创新药带来的对公平和效率的质疑；二是能够突破现有的医保筹资路径，充分调动社会资源参与保障罕见病用药；三是能够进一步提高公众对罕见病的认知，同时有效助力创新药的本土研发。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e多位专家认为，需要总结地方试点的经验，从下而上推动国家级罕见病专项基金的建立。但是，从实际情况看，当前各省市的经济条件、整体管理能力和管理水平差异较大。黄鸣鼓励有条件的地区先把罕见病专项基金建立起来。当很多地方有了成熟经验后，她相信国家层面一定会重视这个问题。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e对于专项基金的资助范围，黄鸣的初步考虑是，一方面针对药物尚未纳入医保的患者群体，使其有药可医、有药可控；也可针对药物已纳入医保、但个人支付比例很高或难以承受的患者群体。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e黄鸣还提到，专项基金能起到核心枢纽的作用，比如，在上海，专项基金能对现有的沪惠保、基本医保、少儿住院基金、医疗救助及残障资助等各类保障资源进行有机整合。通过这种整合，能让原本分散的政策形成合力，从而发挥出更大的保障效应。\u003c/p\u003e\u003cp style=\"text-align: left\"\u003e上海市罕见病防治基金会理事长、中国医师协会儿科医师分会会长黄国英告诉经济观察报，罕见病不仅是医学层面的诊疗问题，更是深度的社会和民生问题。常人往往难以想象罕见病群体所面临的艰辛，这种痛苦不局限于患者个人的病痛，更会波及整个家庭、亲属乃至其所在的社区。因此，这需要多方力量的共同托举。“我们的终极目标，是希望通过全社会的共同努力，帮助罕见病患者打破隔离，真正回归社会，成为社会大家庭中的一员。”\u003c!--MID_AD_4--\u003e\u003c!--EOP_4--\u003e\u003c/p\u003e\u003c!--PARAGRAPH_4--\u003e\u003cp left\"\u003e建立罕见病专项基金最关键的问题是钱从哪里来。王雪静认为，政府层面应该作为主力军，国家医保改革的资金结余、政策定向拨款、国家彩票公益金等都可以成为基金款项来源。在保障范围方面，应有科学的筛选机制，决定优先保障哪些病，确保每一笔钱能合理地使用到患者身上。\u003c/p\u003e\u003cp style=\"text-align:
